What We Do
One Bexley is a partnership of 8 local charities united to improve the lives of Bexley residents. While each charity supports the community individually, we believe that by working together, we can achieve far more for local people than we could alone.
We are dedicated to making a real difference in the lives of local people by working together to provide a range of support and services that truly meet the diverse needs of our community.
We work to amplify the voice of our community and create lasting change in Bexley so that everyone has access to the services and support they need to thrive.

OneBexley Pathways
We are working with local people who may need care and support to carry out adult social care assessments or reviews in partnership with Bexley Council. We will work with you to talk about your interests, things that are important to you and any support that you may need. Then we work with you to find care and support options that are right for you and help you to access them.
Where possible we work with people who are already known to our charities to build on the relationship we already have and make assessments or reviews more personal for you. However, we do also work with people who haven’t previously used our services and aim to build a relationship with you so that you have somebody to come back to, should your circumstances change.
Who We Can Support
We work with adults over 18 who live in the London Borough of Bexley. You could access OneBexley Pathways if:
- You are an older person or
- You have a long-term condition or
- You have a physical or learning disability or
- You need support with your mental wellbeing or
- If you feel you need extra help or support at home or to do the things you enjoy or
- You provide unpaid care for somebody who can’t manage without you (you can still access our support if you do not live in Bexley but the person you care for does)
What we can do
We will talk with you to find what support you need and what would work for you then help you to find the right support or services for you.
If you are eligible for social care support we will work with you to create a support plan which could include:
- Help at home with washing, dressing and meals
- Having a personal assistant or support worker who can go out with you to join activities or do things that you enjoy
- Attending a day centre
- Having a council managed service such as home care who visit you at set times of the day
- Getting equipment that could help you at home, such as a perching stool, hand rails or sensors (for large adaptations we can refer you to the council team)
There are a few things that we’re not able to provide support with but can signpost you to relevant organisations if needed. These include:
- Arranging care home or residential placements
- Safeguarding referrals if you have a concern about somebody
- Housing issues or applications

How to access OneBexley Pathways
If you have already had support or used services from one of our charities before
If you feel that you need additional care and support, please get in touch. You can contact the charity directly to talk through your needs and we will work with you to identify the best solutions for you.
If you are already part of one of our charities, we may be in touch with you to let you know that your annual review is due and offer to complete that with you.

(Please note that as this is a council funded service and you will access services through London Borough of Bexley we will share referral information with them. There may be times that we will refer you to the council to complete your assessment if you need services which we aren’t able to provide)
